General Wrap Around Case Management
Wrap Around Case Management services are available to every child and family receiving support from The Children's Cabinet. Our approach emphasizes strengths, community support, and sustainable, long-term solutions, facilitated through team-based decision-making. These services encompass child and family advocacy, crisis intervention, basic needs security, general support, and streamlined access to resources and referrals. Case Managers frequently collaborate with local public service agencies and other nonprofits to address the full spectrum of family needs. Our wrap-around case management model prioritizes four key elements: 1) Family-directed, 2) Driven by underlying needs, 3) Rooted in a strengths-based approach, and 4) Supported by an effective team process. Case Managers are also responsible for receiving, tracking, and managing all referral data accurately.
SAFE@Home Family First Case Management
The SAFE@Home, Family First Case Manager is responsible for providing comprehensive support to DCFS families referred to The Children's Cabinet. This involves collaborating with Division of Child & Family Services (DCFS) to facilitate the implementation of safety plans, providing focused safety management and services, and coordinating comprehensive wraparound case management. The Case Manager will actively engage in direct outreach and support, providing families in rural Nevada with information, referrals, and resources tailored to meet their unique needs. Key responsibilities also involve thorough documentation of all client interactions, ensuring accurate records that reflect case progress and compliance with safety standards. This role is essential in strengthening family stability, unification and ensuring the effectiveness of safety plans in communities that may face barriers to access due to geographic or resource limitations.
Supervision:
This position has no direct supervisory responsibilities.
SAFE@Home, Family First Case Manager will perform all duties assigned to General Case Mangers as well as provide the high-quality care provided in partnership with DCFS.
Duties of Wrap Around Case Management:
Understanding the Agency and Programs
- Develop a thorough understanding of the agency’s mission, strategic focus areas, and overall objectives, actively contributing to their success.
- Read, understand, and consistently apply all agency and department policies and procedures.
- Maintain a comprehensive knowledge of agency programs and community resources to provide accurate information and referrals to families during case management.
- Participate in program-related activities, including staff and parent meetings, to stay engaged and informed.
- Attend Children’s Cabinet-sponsored events and activities as requested (e.g., ART of Childhood, field trips, graduations, and awards ceremonies).
Comprehensive Case Management
- Conduct intake assessments to evaluate clients' needs, strengths, and goals, including the family
- Develop individualized service plans with clear and achievable objectives, monitoring progress and adjusting as needs evolve.
- Provide wrap around case management services to families and youth involved in Cabinet programs, ensuring tailored support based on individual circumstances.
- Conduct home visits and field activities to evaluate clients' needs within their own environments, ensuring personalized case management support is delivered when and where it is most needed.
Advocacy and Client Empowerment
- Serve as an advocate for clients, assisting them in accessing essential community resources and services.
- Guide clients through various systems, including healthcare, social services, housing, and education.
- Empower clients to become self-sufficient by encouraging self-advocacy and the use of available resources.
Crisis Intervention and Support
- Respond to crises, providing immediate assistance, emotional support, and solutions as needed.
- Connect clients with emergency resources, such as shelters, food assistance, and mental health services, while offering de-escalation support during high-stress situations.
Access to Resources and Referrals
- Identify and connect clients with appropriate resources for basic needs, mental health, employment, and education.
- Maintain a network of current community resources and referral options, educating clients about available services and supporting them throughout the referral process.
- Actively engage in community events and outreach activities that support The Children's Cabinet's mission and help connect clients with additional resources.
Data Management and Reporting
- Accurately document client interactions, service plans, and progress in case management databases.
- Utilize case management software to track data, monitor outcomes, and ensure all information is kept confidential and compliant with data privacy standards.
- Prepare and submit monthly reports as directed to support program evaluation and accountability.
Regular Case Reviews and Team Collaboration
- Participate in team-based case reviews to assess client progress, making adjustments to service plans as necessary.
- Prepare case summaries for review meetings and contribute to the team’s service planning.
Eligibility and Transportation Assistance
- Assess eligibility for youth participating in various programs and, when needed, provide transportation support for families and youth to attend required appointments or activities.
Professional Development and Organizational Protocols
- Participate in in-service training and other staff development opportunities to maintain and enhance professional skills.
- Provide regular updates to supervisors on case progress and any notable issues.
- Adhere to all organizational policies, ethical standards, and applicable regulations, supporting the agency’s mission and enhancing service delivery.

SAFE@Home, Family First Case Manager will perform all duties assigned to General Case Mangers as well as:
Implements the Family First Safety Management Program:
- Executes the Family First Safety Management framework by adhering to established protocols and procedures to ensure family safety and support.
Provides Comprehensive Information and Referrals:
- Offers tailored information and connects families with relevant community services and resources, aligning support with family needs and program goals.
- Assesses the specific needs of youth and families, making referrals to appropriate community resources and providing direct support, including emergency and essential items as needed.
Delivers Direct Safety Services and Wrap around Case Management:
- Engages families in direct safety interventions and manages ongoing safety services to address immediate risks and promote family stability.
- Provides structured case management to families engaged with DCFS, ensuring alignment with program objectives and DCFS requirements.
- Maintains Client Contact as Outlined in Safety Plans
- Engages in regular contact with clients and families as specified in DCFS safety plans, unless otherwise directed, to monitor progress and ensure safety standards are upheld.
- Attends weekly supervision meetings to review case progress, obtain guidance, and collaborate on service strategies.
- Actively participates in safety plan determination sessions with DCFS staff, following the guidelines set forth in policies and procedures.
- Documents Case Notes in UNITY: Accurately enters all case notes and updates into UNITY, Nevada’s SACWIS system, to ensure comprehensive record-keeping and compliance.
Prepares and Submits Reports:
- Compiles, reviews, and submits required reports to ensure program accountability and track progress toward goals.
Other Duties as Assigned
- Perform additional duties as assigned to support the agency’s mission and contribute to the overall success of its programs.
Minimum Qualifications:
Knowledge of
- Appropriate interactions with children and families;
- Community agencies and resources, as well as an understanding of the problems facing youth and families;
- Positive parenting concepts and work/life supports for families;
- Family and youth relationship issues;
- Sound decision-making, crisis management techniques, be self-motivated and a team player;
- Case Management skills with an emphasis on wraparound and facilitating services;
- Computer applications and the desire to learn new programs.

- Keep appointments punctually, be organized and demonstrate follow through;
- Express ideas clearly and concisely both orally and in writing;
- Adhere to schedules and specific timetables;
- Use good organizational skills and the ability to be task-oriented;

- Be physically capable of performing all job duties, including; being able to lift up to 40lbs, walking up or down stairs, and sitting at a computer for long periods of time.

- Work as a member of a multidisciplinary team.
Special Requirements
- Able to pass complete background check.
- Bachelor degree preferred in Related Field or equivalent combination of education and experience in early childhood through adolescence.
- Bilingual (English/Spanish) preferred.
- Must have reliable vehicle for work and clean driving record, home visits, rural community travel, and training.
- May be required to travel in state from Elko to Winnemucca and other areas as needed for client home visits, meetings, or training.
- Must possess a valid Nevada Driver's License, and proof of insurance for utilizing a personal vehicle for company business.
- Work requires some weekend and evening hours.
- Be physically capable of performing all job duties. This may include lifting and carrying up to 40 lbs., walking up and down stairs, and data entry requiring repetitive typing motion.
