General Wraparound Case Management
Wraparound Case Management services are available to every child and family receiving support from The Children's Cabinet. Our approach emphasizes strengths, community support, and sustainable, long-term solutions, facilitated through team-based decision-making. These services encompass child and family advocacy, crisis intervention, basic needs security, general support, and streamlined access to resources through closed-loop referrals. Case Managers frequently collaborate with local public service agencies and other nonprofits to address the full spectrum of family needs. Our wrap-around case management model prioritizes four key elements: 1) Family-directed, 2) Driven by underlying needs, 3) Rooted in a strengths-based approach, and 4) Supported by an effective team process.
Safe Place Outreach Case Manager
The Safe Place Outreach Case Manager provides crisis intervention, case management, referrals, and direct services to youth and families referred to The Children’s Cabinet. This includes answering the Safe Place Hotline, conducting in-person interventions, coordinating shelter placement, and supporting youth through critical transitions. The Safe Place Outreach Case Manager also plays a key role in outreach efforts to engage community partners and ensure that vulnerable youth can access safe and supportive resources.
Supervision: This position has no direct supervisory responsibilities.

Duties & Responsibilities
The Safe Place Outreach Case Manager will perform all duties assigned to General Wraparound Case Managers, as well as the following:
Crisis Intervention & Direct Support
- Conduct crisis intervention triage interviews with callers to gather sufficient information and determine appropriate responses.
- Provide face-to-face crisis intervention when necessary, responding in person within a reasonable timeframe.
- Arrange safe transportation for youth to emergency shelters, if deemed necessary.
- Conduct comprehensive intake assessments, prioritizing the most urgent needs of youth.
- Facilitate smooth transitions for youth entering shelter, ensuring they receive appropriate support.
- Complete all required documentation, including CPS reports, police reports, and health screenings, when appropriate.
- Maintain timely and accurate case notes in case management software to ensure proper tracking and compliance.
Safe Place Program Operations
- Answer the Safe Place Hotline during scheduled shifts, ensuring 24/7 coverage through on-call rotations.
- Ensure the Safe Place Hotline is transferred appropriately during shift changes to maintain continuous access for youth.
- Collaborate with emergency shelters, law enforcement, and other community partners to ensure coordinated services for at-risk youth.
- Request Juvenile Court Automated Tracking System (JCATS) reports for youth seeking shelter services.
Community Outreach & Prevention
- Conduct targeted outreach to engage youth and families in need of services.
- Educate youth, school staff, and community partners about the Safe Place program and its services.
- Maintain strong relationships with existing Safe Place sites and identify new potential Safe Place partners.
- Participate in community events to promote Safe Place services and increase accessibility.
Collaboration & Reporting
- Consult with the Program Coordinator
on every Safe Place call to ensure appropriate response and decision-making.
- Attend weekly supervision meetings
to review case progress, obtain guidance, and collaborate on service strategies.
- Actively participate in case reviews to assess youth progress and adjust intervention strategies.
- Submit monthly reports on program activities, client outcomes, and outreach efforts as directed.
- Stay up to date with best practices in trauma-informed care and crisis intervention through ongoing professional development.
Minimum Qualifications
Knowledge of:
- Crisis intervention techniques and trauma-informed care principles.
- Community agencies and resources for youth and families in crisis.
- Positive youth development strategies.
- Family dynamics and the impact of abuse, neglect, and homelessness.
- Case management principles, including wraparound services and referrals.
- Ethical considerations when working with vulnerable populations.
Ability to:
- Provide rapid and effective crisis intervention services.
- Express ideas clearly and concisely both orally and in writing.
- Work effectively with families from diverse backgrounds.
- Problem-solve in high-pressure situations and make sound decisions in crises.
- Use case management software to document interactions and track client progress.
- Maintain professionalism and confidentiality in sensitive situations.
- Work flexible hours, including evenings and weekends as needed.
Special Requirements
- Bachelor's degree preferred in a related field, or an equivalent combination of education and experience in youth services.
- Bilingual (English/Spanish) preferred.
- Must have reliable vehicle for work-related travel.
- Must possess a valid Nevada Driver’s License and proof of insurance if utilizing a personal vehicle for company business.
- Must pass a comprehensive background check.
- Work requires some evening and weekend hours.
- Must be physically capable of performing job duties, including lifting up to 40 lbs. and responding to crisis situations in the field.
