FAMILY SUPPORT NAVIGATOR

Duties

Family Support Navigator will perform all duties assigned to General Case managers as well as:

  • Complete intake with parents to achieve a comprehensive understanding of the unique needs of the family.
  • Provide mental health referrals and/or enhanced referrals via warm hand-offs utilizing knowledge of community resources, database programs, and other information sources.
  • Work with family to understand their insurance coverage options. If uninsured, provide the family with information on Medicaid and assist with the application.
  • Assist families in navigating resources and support that meet their needs.
  • Support families understanding eligibility requirements of various mental health support programs and how to access the required documentation if applicable.
  • Assist families in selecting mental healthcare providers by providing information on types of licensure and professional qualifications, insurance requirements, and waitlist timeframes.
  • Assist clients with scheduling and coordinating medical and behavioral health appointments
  • Work with other Children's Cabinet services and community partners, including the Washoe County School District, to create a closed referral system to ensure families' needs are met.
  • Use identified tools and assessments to serve families, including Columbia-Suicide Severity Rating Scale
  • Use appropriate interpretation to support families whose first language is not English.
  • Follow up with all clients on time to make sure needs are met.
  • Track all contact and support given to families in the identified data system.
  • Stay informed about new and existing resources and services in the community
  • Participate in the development of consumer education materials (newsletters, tip sheets, calendars, brochures, etc.) based on the information needs of families.
  • Perform other tasks as assigned.
  • Performs other duties as assigned.

Minimum Qualifications:

Knowledge of

  • Community services and supports for families including mental health providers and health insurance eligibility.
  • Diversity, Equity, and Inclusion principles
  • Trauma Informed Care practice principles
  • Positive parenting concepts and work/life supports for families.
  • Customer service skills and positive communication styles
  • Computer programs including Microsoft Word, Excel, Power Point, database programs, etc., and the desire to learn.

Ability to:

  • Communicate clearly and concisely both in writing and verbally.
  • Be organized and use effective time management.
  • Facilitate positive, strength-based communication with clients.
  • Work collaboratively with staff and community agencies.
  • Maintain confidentiality of client/agency information
  • Understand and follow oral and written instructions.
  • Work with adults and youth of diverse backgrounds and beliefs and adapt service delivery accordingly.
  • Be able to understand and relate to clients' experiences and emotions and the complexities of mental health issues.

Job Requirements:

  • Bachelor's degree in human services field or 4 years of experience in human services setting, community service setting, or case management position
  • Bilingual (English/Spanish)  
  • Experience in Mental Health services.
  • Must have reliable transportation for work and possess a valid Nevada's Driver's License and proof of insurance for utilizing a personal vehicle for company business
  • Must be able to lift and carry up to 25 pounds.
  • Must be able to walk up and down stairs.
