Definition

The Subsidy Wraparound Case Manager is a specialist level position who is responsible for developing support plans with families in an effort to assist clients with complex referral and service needs in addition to subsidy eligibility. The Wraparound Case Manager will use a team-based framework to assist clients with any eligibility requirements, referrals to outside agencies, and will continue to meet with families to achieve the goals they set in their plans.

Duties

  • Achieve an understanding of agency mission, strategic focus areas and overall goals and objectives. Contribute to the success of these areas.
  • Read, understand, and apply all agency and department policy and procedures set forth in both the Agency Handbook and Program Manual.
  • Have a complete understanding of the agency's programs and community resources in order to provide information and referrals to families through case management services.
  • Conduct intake assessments to evaluate clients' needs, strengths, and goals, including the family.
  • Develop individualized service plans based on family strengths with clear and achievable objectives, monitoring progress and adjusting as needs evolve.
  • Provide wraparound case management services to families enrolled in the child care subsidy program or on the wait list, ensuring tailored support based on individual circumstances.
  • Assist Subsidy clients with emergency needs requests and rental assistance requests when available.
  • Guide clients through various systems, including healthcare, social services, housing, and education.
  • Empower clients to become self-sufficient by encouraging self-advocacy and the use of available resources.
  • Respond to crises, providing immediate assistance, emotional support, and solutions as needed.
  • Assist families in their selection of child care by providing information on quality child care and referrals to licensed child care options that meet their individual needs. This includes daily care and respite care.
  • Accept and return phone calls from families requesting childcare services provide program guidelines, assist with prescreening and application process.
  • Collect and maintain statistics necessary for program compliance, enrollment options and reports.
  • Utilize case management software to track data, monitor outcomes, and ensure all information is kept confidential and compliant with data privacy standards.
  • Prepare and submit monthly reports as directed to support program evaluation and accountability.
  • Prepare case summaries for review meetings and participate in team-based case reviews to assess client progress and adjust service plans as necessary.
  • Assist parents with complaints about program and/or child care services, mediate as needed, and advise of proper procedure for resolution of grievances.
  • Perform travel related duties as needed to ensure adequate administration of the program.
  • Performs other tasks as assigned.

Special Requirements

  • Bachelor degree in related field or equivalent experience in social work and/or early childhood education.
  • At least three years of experience in community service or case management position
  • Able to work with clients in crisis situations
  • Bilingual (English/Spanish) preferred.
  • Must have reliable vehicle for work and travel between offices and community events
  • Must possess a valid Nevada Driver's License and proof of insurance for utilizing a personal vehicle for company business
  • Be physically capable of performing all job duties. This may include lifting up to 40 lbs. Requires repetitive key stroking; computer work; standing, sitting, and walking; stairs
  • Work requires some weekend and evening hours.
  • Requires some travel to other Offices and may include training within and outside of the state.
  • Able to pass complete background check.
